Aged Care Facilitator

Transition an elderly loved one into aged care can be an emotional and stressful time. At Page Care Services, we can act as an Aged Care Facilitator between the family and the elderly loved one to discuss options.

What does this involve?

This service is generally provided to the loved one in the comfort, safety and privacy of their home to ensure there is little or no anguish, stress or pressure to make a decision.

We require other family members to attend if convenient.

It is simply a discussion to ensure your loved one understands that there are some alternatives to living at home that will ensure that their safety and 24/7 care is a priority and is achievable.

Provide the peace of mind to your loved one that both the family and the aged care specialist are working together to ensure they feel safe, happy and cared for.
